New Patients With Dental Insurance:

We'll be happy to file your insurance for your first visit. Please be prepared to give us the information we need when you call to make an appointment. We'll need the name of the insurance company, the policy holder's name, date of birth, ID number and the name of the employer. It's also helpful if you can fax a copy of the card to us. If we can verify coverage before your appointment you'll just owe your co-pay when you come in.

If we are unable to verify coverage before your appointment we'll ask that you pay in full for the initial visit. We'll then file your insurance and reimburse you (minus any co-pays and out-of-pocket expenses) when we receive payment from the insurance company.

Insurance Claims:

In order to file your claim, we must have a copy of your insurance card. As a courtesy to our patients, we will bill your insurance company for services and allow them 60 days to render payment. After that, you are responsible for the entire balance, paid-in-full.

New Patients Without Dental Insurance:

If you do not have dental insurance, payment is expected at the time dental care is received. We accept MasterCard, Visa, American Express and Discover. We also accept Care Credit — a third party financed, no interest payment option.
